What service or system do I require to use my laptop while out and about,
without relying on wireless hotspots? Or is that not possible, or
ridiculously expensive?

GPRS, but you need to define "ridiculous"

You need a receiver (either plugged into the PC card slot, into a port,
or cell phone paired with Bluetooth) and service.
It won't be fast, it won't be as simple to connect, but it can happen
anywhere.

It might be more practical (and certainly will be easier) to subscribe
to one of the coalition services; services that tie hotspots together
so that a user can connect to groups of different coalition members.

As mention'd GPRS.
I have had my Nokia phone connected to my computer with Bluetooth and
connect to the internet as a 56K modem (thru the phone).
