I now just have: Explorer, Systray, Loadqm, loading up. And I still have the
same problem.

If I haven't shut the PC down in the correct way, scan disk normally pops up
before I log on to my desktop, before any programs have loaded, so I don't
see how that can be causing it.

I don't know what else to try.

Just in passing ...
You need at least 15% of the HD free before defrag.
Defrag in Safe Mode.
Turn off the screen saver.
Scan disc.

You're very welcome. I also have the same problem ... can't defrag unless I use Safe mode.

Defragging in safe mode works fine. I just don't understand why this
problem has suddenly occurred when nothing has really changed on my PC.
