still image capture with EXIF data from DV-AVI video?

Is there a program that does "smart" still image capture from DV-AVI video,
saves the embedded date/time data from the DV-AVI video stream, and saves it
as EXIF data in the captured still image file?

That would be useful but I doubt that anyone's done this.

DV video and DV-AVI have timecode embedded, but not date and time of shoot.
Any NLE program will export a frame as a bitmap picture. When doing so name
it according to time and date of shoot. No program would "capture" a frame
from a DV-AVI because an AVI, whether DV or not, is a file. The video on a
DV tape is not an AVI, nor is it an MOV. These are the envelopes which hold
the video data particulars.

So, you will have to search for the frame you want and be the "smart image
capturer".
