I feel your pain.

I am sure it is a bug in stream.sys, however I failed to convince MS to look
at it.
A few years back when all my development was Stream Class based I sumbled
upon the problem when sometimes callbacks scheduled from callbacks were
getting lost.
It sounds like you are seeing the same problem.

I reported it to MS but the bug was closed with beloved "can not repro"
resolution.
At about the same time I started switching to AVStream and this issue
stopped being a problem for me.

Somehow I feel that you won't get MS to fix it. Especially now as Stream
Class is being deprecated.
