Does a graphic designer need permission from a customer to use something
they designed for the customer in an advertisement? Does a printer need
permission from a customer to use a sample of a printed piece in an
advertisement?

the answer: Depends
As a rule of thumb inclusion in the artist portfolio is a given. The
question in my mind: is an advertisement a logical extension of the artist
portfolio?
A lot would depend on the wording of the contract between the artist and the
customer. I would expect if the customer was Disney it would be an absolute
no no OTOH work for a local business would likely be an accepted practice.
Can you give more details?

the copyright to the created item was transferred to the client, or other
restrictions were written down, you have the right to use the item as you
wish. Without the transfer in writing, the copyright belongs to you.
In real terms, a lot may depend on if you would like to continue working
with this client, if the outside use will sour your relationship.

Just to cover my own ass I always include the following in my project
agreement:
"The designer retains personal rights to use the completed project and
any preliminary designs for the purpose of design competitions, future
publications on design, educational purposes and the marketing of the
designer's business. Where applicable the client will be given any
necessary credit for usage of the project elements."
As a professional courtesy I would think that any printer should ask
both the client and designer before using a piece they printed in their
own marketing materials.

purchasing images (e.g. from some site like gettyimage) on behalf of
customers?

Katy, be wary of COST, and how much you mark it up to the client.
Make sure you get paid in advance of purchasing the image.
Make sure you know EXACTLY what the licenses are on the images you are
buying (ie. Print, web, signs).
