Peter, we (and some but not all other ISPs) can place something called
a "Combo" order.
This is a combined "new PSTN line" order and "ADSL" order, and
*speciifically* addresses the problem you have - if the line won't
work for ADSL, you don't keep it or pay for it.
More-over, whilst it's early days on the new no-limits system, we are
hopful that the combination of a combo and a no-limits "try it and
see" distance means they'll put it in with this in mind and already
seek out the shortest cable run available (etc) to see if they can get
it all to work properly.

Michael,
we're another ISP who can handle a "combined" order, and are happy to
do so.
More-over, since the introduction of the new distance and noise rules
on Monday 6th, you may like to try again, you may find it'll work out
this time. And as before, if it doesn't, it costs you nothing bar a
little time and effort.

No, ISDN2 (ISDN2e, Home Highway, Business Highway) is implemented over
a *single* copper pair. If there's a second pair into the house
already (quite common) then it will be available for a new PSTN.
However, this isn't necessarily important - if you order a new PSTN,
BT are unlikely to turn you down, rather you may just have to wait a
few days whilst they run fresh copper from the pole.
