Audio device missing - no sound plays

Somehow my audio device disappeared! When I look under the control
panel/sounds and audio device properties, under the Volume tab I see "no
audio device". However when I look under the Hardware tab I see the correct
audio device which is said to have a good driver and be functioning properly.
What determines the default audio device and how can I change it?

Have you tried a reboot, I've had this happen twice and a simple reboot
brought it all back.

Rebooting has no effect on this behavior. It persists.
