- Strange Behaviour

Since last week while working away on my system some icons in the taskbar
will look like 8-bit colour (mainly the Microsoft Outlook icon) when this
happens I can not click any icons on the desktop nor can I access the
control panel and the only way to fix the problem until it happens again is
to shutdown the system and restart, I can not do this by going to
Start/Shutdown, the only way is to Ctrl/Alt/Delete and shutdown that way.
When this problem happens I try click on icons on the desktop etc and
nothing happens, sometimes I get a message saying "There is not enough free
memory to run this program. Quit one or more programs and try again"
I have 2GB of memory and a video card of 256MB, CPU of 3.4GHz, so I'm not
really sure if this is a hardware or software problem. I have reinstalled
the video drivers using the latest version, BIOS is updated to the latest
version.
Please help me out here.
I am due to format and reinstall windows if need be

Also after restarting the quick launch toolbar has disappeared
