When I scan a document, then print-
The document is the same size 1:1
If I save the document, and set the image to be any size
(whether 1:1 or smaller) the image is 4X or larger.
What is causing the image to drastically increase in size upon saving
it to file?

Lost of the DPI value.
Printing depends on the DPI information to know what the image size
represents.
A 300 DPI scan of a 11 X 8.5 inch paper results in a 3300 x 2550 pixel image
file.
If you print that file at 300 DPI you get the 11 x 8.5 inch print. If you
print that file at 100 DPI, you get a 33 x 25.5 inch print.
