Lines and boxes are not text, and wont be retained by the OCR software.
I think you are saying that you want to retain them, as a "form", and
this will be a problem for conventional OCR software (which is not form
oriented).
There is another category called Forms Software, designed for forms.
OmniForm is a popular example. I have not used it, but I think it would
obviously be more what you need for actual "forms".

I may not understand, but if you might only have a few such forms to do,
and since you want a Spanish translation anyway,
which implies that you must retype every character anyway,
then it might be as easy or easier to simply manually create duplicate
forms in Word from scratch, if Word will do what you want.
