Make sure you get SP6a. (I doubt that the original SP6 is still available,
but you never know.) Then there are a ton of security patches and hotfixes
to apply post SP6a, most of which will require reboots. Once you've applied
SP6a, you can then use Windows Update to scan your system and it'll give you
an idea which fixes you need, but don't use WinUp to install/apply them.
Note them and get them manually. There have been a few reports of system
malfunctions as people have installed these, so take your time with them and
do regular backups so you can do a restore if you get glitched.

Of particular note is the very latest hotfix, 823803, which breaks RAS and
RRAS. There's a version of this hotfix that works that you have to get
through PSS when you're ready for it.

Applying these will take a lot of time and energy, and you'll be putting
that time and energy into a product that will soon be obsolete and
unsupported. If your organization can afford it, as an alternative, you
might want to consider upgrading to SBS 2000 instead of patching 4.5. I
think you can still get the $99 software assurance deal, so that when
SBS2003 is released (within a couple of months), you can get it and upgrade
relatively cheaply and easily. You have to go through SBS2000 to upgrade to
SBS2003 anyway, so there's no point in waiting for its release.

Last time I checked (here in Mexico) they wouldn't allow software assurance
for SBS2K. Hence I am waiting for SBS2003 to come out.
