website info / history utility?

This seems impossible, but I'm looking for a utility that would let me keep
track of the following information from websites I visit:

1. login id
2. password
3. e-mail address that I provided to website to use it
4. other information provided to website (name, security question/answer)
5. history of my visits to site
6. titles and URLs of pages I looked at, and when
7. my own notes about website/pages
8. works with Mozilla, preferably

Moz gathers a lot of this stuff with history and password manager, but I
don't know of any way to extract it to human readable form.

Freeware preferred, of course, but if you know of a payware program that can
do all this, I'd appreciate an e-mail to:

Seems to me that if you install some sort of spy software, the kind that
tracks your kids or employeees, you'd get the reports you're looking
for.
