Is it safe to delete old hotfixes and service pack backups from the winnt
directory to free up space
once they are deemed not necessary ?
Will deleting them manually remove their reference from add/remove programs.
What is the proper cleanup procedure?

Safe to delete, not sure if they will be purged from the Add/Remove. But if you click on them and the uninstall information is not
there Add/Remove will propmt to ask if you'd like it removed.

You can delete the directories
%windir%\$KBxxxxxx$
If you attempt Add/Remove on a program that can't be found, you should be
prompted that the entry can be removed. Else navigate to
HKLM\Software\Microsoft\Windows\CurrentVersion\Uni nstall
and delete the subkey for the program to remove it from Control
Panel|Add/Remove Programs
