I formatted Maxtor 30Gig for fresh install on P3 450 which I have done many
times but have never seen this:

Report:

Standard Mode: Bad fault in MS-Dos extender.
Fault: 000D Stack Dump: 0000 0000 0070
Raw fault frame: EC=0000 IP=6069 CS=046F FL=3212 SP=0000 SS=03cF

Could anyone help with a clue?

It's a good bet that your RAM is defective, replace it and run setup
again.
