1 to many Relationship displayed in a form

On my contacts form, I want to be able to track the contact's favorite color
and the year it was their favorite color. This will be a one to many
relationship, one contact to many colors/years.

I created a custom entity: "ContactColor"
I added 2 attributes of "color" and "year"

I opened up the contact form and added the relationship, but how do I added
these fields to my form?

As a contact has 0 or more contact colors, they are not available on the
contact form. You should see a new link in the left navigation bar showing
the display collection name of your custom entity. Click on it and you see
the associated contact colors view.

You can add the contact lookup field (probably new_contactid) to the contact
color form though.
