- Error encountered when converting Lead to Opportunity - now Lead is "lost"

A Sales user encountered a Server Application error when converting a
Lead and she is now unable to find the Lead or the Account. I have
verified it is not listed in Leads or Accounts, but I do not know where
or how to search the database tables to find out where is is still
stored at.
Any help would be appreciated!

Have you looked at the "Closed Leads" view, or used Advanced Find to query
for leads modified today (leave the status out of the query). You can also
look for accounts created today to see if it got created as something
unexpected.
