MS CRM 3.0 Toolbars

One of my customers has CRM 3.0 installed, including all service packs.
However, in the CRM desktop client for Outlook, the CRM toolbar currently
displayed has the name Custom1. Each time Outlook is started, a new CRM
toolbar is created and displayed. For example, at the moment, she has Custom1
and Custom2.

Earlier today I deleted all the customX toolbars, numbering over 90. I then
uninstalled the desktop client, rebooted, and re-installed the desktop
client.

When she opened Outlook, it showed one toolbar for CRM, named Custom1. She
exited Outlook, and then started it again. Outlook displayed two CRM
toolbars, Custom1 and Custom2.

I searched the registry looking for references to these toolbars without
success.

How can I make the original CRM toolbar reappear and these "custom" toolbars
disappear permanently?
