Multiple Campaign response forms - 1 per active Campaign

CRM should allow the creation of a specific Campaign Response form for each
active Campaign, if required.

We run multiple campaigns (DM, advertising, phone etc). For each campaign we
may want to gather specific information - more than just a 'yes' or 'no'. We
can easily add new attributes to allow this, but have to use JavaScript to
show/hide fields based on the Campaign - this only being selected once the
form is displayed.

How much easier it would be if this definition could be done at the campaign
level!
