How can you add additional organizations to a CRM server?

I've looked through all the documentation I can find and
haven't seen it. Sorry if this is something really
obvious, I have yet to find how to do it.

Technically, you can't. However, you could implement something like this
via the Business Units.

Could you do that using different databases for each as
well?

You can only have 1 CRM install per IIS server and it is tied to an org
name.
