I don't know if you did this, but you need to restart IIS after
publishing your customization.

I have seen this before ..again its a caching issue i think and restarting
iis by opening up a command prompt and type iisreset will do the trick

I tried adding the "territorycode" field to the Account form, and I tried
adding a tab to the Account form. Both worked fine, and they appear in the
previews correctly. However, when I try to open the forms normally (to
view, edit, or enter an account) none of my form modifications appear. I
went back and double-checked that the form modifications were saved, and
they indeed were all still there in the customization section. I tried
restarting IIS (twice even) with no success. Any ideas?

After making modifications, you need to:
#1. Publish the affected object's schema in the deployment manager
#2. Then, restart IIS
