Been a while since I've been on, but it's good to be back.
Here's the deal:

I've got a custom entity in CRM which has a many-to-one relationship with
the Account entity. To this custom entity I have created a many-to-one
relationship with the Incident entity, with the custom entity's accountid
lookup attribute mapping to the customerid attribute of the Incident.

When I create a new Incident (case) from the custom entity's Incident list
view the relationship seems to be set correctly - the Account to which the
custom entity belongs appears in the new Incident's "Customer" field.

Unfortunately, when I try to save the new Incident, I get an error dialog
"An error has occurred, for more information...."

Below is the stack trace from the CRM trace log:

Jon, we had something similar a few months ago - same issue with mapping to
cases was causing some fireworks.

At the time, MBS gave us a private hotfix, but my understanding is that it
has been rolled into Rollup Patch 2 - have you installed it yet?

The KB article (in customer/partner source) was 927205 if you want to read
about it. Although, looking back at the KB, it doesn't describe all the
problems caused by the bug. I'd suggest tryng Rollup2 and see if that helps
