Sales for Outlook 1.2 Client Install Errors

We have been experiencing some problems reinstalling the
Outlook Sales client on a machine that lost functionality
with the CRM Appointment button. The form just stopped
loading when the button was clicked, so we followed the
following steps:

1. We removed and then re-added the COM object Microsoft
CRM Outlook Addin and also cleared the forms cache in
Forms Manager. This did not resolve the issue.

2. We then completely uninstalled Sales for Outlook 1.2,
rebooted the system, reinstalled and rebooted and when
Outlook was launched, it now delivers the error "An error
occurred installing Microsoft CRM folders". None of the
CRM folders are now avaiable in the client.

3. We then reperformed the uninstall-reinstall process
above and also completely removed the MS SQL installation
on the local machine and deleted all remaining folders
from the local drive. Rebooted. Reinstalled. Rebooted.
Same message. "An error occurred installing Microsoft CRM
folders".

4. I just went to the length of uninstalling Outlook as
well as the CRM client and SQL and doing a full reinstall
of all three items. The outlook version is Outlook 2003.
After all of the reinstalls and reboots, same error. "An
error occurred installing Microsoft CRM folders".

I am at my wits end on this. Calls to Microsoft tech
support on our behalf by our consultants have resulted in
much head scratching to date, so I am interested in
fielding any suggestions that anybody else might have on
this.

Has anyone else run across this problem? If so, was there
ever any resolution outside of reformatting the entire
system? I just don't want to go to that extreme if I can
avoid it, as I now have two such affected systems and a
third that has now also exhibited the malfunction of the
CRM Appointment button.
