I have a task. I need to receive e-mail reminder 5 days after task is
completed.

Unfortunatelly MS Dynamics 4.0 does not work properly, because the workflow
that should send the e-mail, send it 30 minutes after the task is completed
!! In start and actual start date for the e-mail(that should be send from the
workflow) I had put down "5 days after task is completed" !

Any suggestions, please.

You getting your condition wrong, i think.

You need to check for the task status is completed and also the second
condition should be a 5 days after the actual end date of the Task. When
building your condition, there should be a Form assistant on the right
hand side, select the number of day and the Actual End Date and the
value should be highlighted in yellow in the corresponding field.

I took me 5 minutes to do it and it's already working fine!
