How does one create a file association for a file that has
no extension? Example... Hosts file to open with NotePad.

You don't.
What you need to do is right-click on it and "Send To" Notepad.
If you don't have Notepad in your "Send To", then goto C:\Windows\SendTo, click File/New/Shortcut, and browse to C:\Windows\Notepad.exe
