DHCP Giving Duplicates Address

In our enviroment, we have DC's at 5 locations. Each DC runs DHCP, and each
DHCP server gives out some duplicate IP adresses. Why does this happen, and
how can we prevent this form happening?

Duplicate to another DHCP server?

Duplicate to existing manually configured machines?

Duplicate to itself? (very unlikely)

For the first two reason the answer is likely that you don't have the
proper EXCLUSIONS configured for the scope. All addresses
that cannet be given by a DHCP server must be excluded.

The last reason isn't even a likely possibility so unless you are sure
that is happening there is not point in guessing on that.

basically each dhcp server is giving out duplicate ip addresses to to some
pc's, this of course happens within each individual site. For instance, site
A the DC gives PC 1 and PC2 the same IP address 192.x.x.12. Site B will do
the same PC3 and PC4 gets the same IP address 172.x.x.4

I have actually seen 1 IP address gieven out to 7 different nodes. There
are some addesses that are not duplicated though. I hope this is enough
information.

I dont remeber configuring exclusions only a dhcp range.
