Server 2003 locks random files

Hi.

We got a huge problem and after several atemps by various employees no
clue of a cause or soulution:

A Win2003 server locks randomly files. Week before the last a certain
PHP-Script got away, wasn't to be found via Explorer or DOS-box or
Search function on the machine. But when we tried to create a file
with that name in the directory where it should be, we received the
message that a file already exists by that name. Later we found out
that on the WHOLE system no file with the phrase 'charset' could be
created: They disappeared right under the mousepointer. Same effect in
the whole machine: When trying to create such a file again, the system
said it already existed.
After a day or two the phenomena vanished.
Right till Monday, when it showed the same effect, now with files by
the name 'java.exe'.
The file has 'survived' in a zip file, but can't be created in a
'normal' folder.
Of course we searched for any virus, trojan, rootkit etc with various
tools but the system seems to be clean.

Any ideas, what could cause such an behavior?

Help would be very much appreciated.
